This form is required if you require:
- Hoarding
- Footpath/Verge Occupation or closure- (including placement of any plant, equipment within the verge, footpath or road)
- Elevated Works Platform (EWP) or Crane (sited on road or footpath)
- Temporary Road Closure
- Case Managed Works Permit- Major Construction site
