Home and Community Care

The Home and Community Care (HACC) program represents the commitment from both City of Prospect and Town of Walkerville to the care and wellbeing of our more vulnerable residents, providing assistance to remain independent, healthy and valued members of our communities.

Home Assist Program

The Home Assist program assists frail, aged, younger people with a disability and Carers to maintain independence in their own homes. An initial assessment in your home will be conducted to explain the program and to determine individual eligibility.

The service provides short-term practical assistance with:-

  • Home Help
  • Garden maintenance
  • Referrals to other agencies e.g. District Nurse, Domiciliary Care, Meals on Wheels etc.
  • Information &Advocacy
  • Essential minor home maintenance
  • Support Services e.g. domestic help following illness or hospital discharge
  • Home Safety and Security Program
  • Limited transport assistance*
  • Carer Support

The Home Assist Program is a partnership between the Commonwealth, State & Local Government.
Client contributions towards costs are appreciated.

HACC Social Programs

The HACC Social programs provide people who are frail, aged, younger people with a disability and Carers, with a chance to mix in a social atmosphere. An initial assessment in your home will be conducted to explain the program and to determine individual eligibility.

The groups include:

  • Friendship Groups
  • Market Run
  • Shangri -la
  • Explorers
  • Men's Shed
  • Lunch Group
  • Exercise Groups
  • Seniors Community Lunch
  • Zoo Group
  • Fishing Group
  • Matinee Group
